These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Boswellia Serrata interacts with whole categories of medication. Chlorpheniramine, Hydrocodone, Pseudoephedrine falls into 2 of those categories, so it can interact with Boswellia Serrata in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Chlorpheniramine, Hydrocodone, Pseudoephedrine
Chlorpheniramine, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cough and cold product has a tricky piece: hydrocodone is a prodrug. Your body uses an enzyme called CYP2D6 to turn part of it into hydromorphone, which adds to the pain relief. In lab dishes, boswellic acids appear to slow that same enzyme down. If that happened in people, you could actually get a bit less conversion to the stronger active form, so the pain or cough relief might feel slightly weaker. On the flip side, parent hydrocodone could linger a little longer, which theoretically nudges up drowsiness or constipation.
Honestly, this is based only on test-tube data, not real patients, so it may not matter at all at normal doses. Still, since this product already causes sleepiness, it's worth a quick chat with your pharmacist before adding boswellia.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Chlorpheniramine, Hydrocodone, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product (Tussend) pairs an antihistamine, the opioid hydrocodone, and the decongestant pseudoephedrine. The piece I want to flag is hydrocodone, which your liver partly processes using an enzyme called CYP3A4. In lab studies, boswellia mostly seems to slow down that enzyme. If that happens in real life, hydrocodone could build up a bit more than usual, which might mean stronger drowsiness, dizziness, constipation, or slower breathing.
That said, this is only a theoretical concern based on test-tube research, not real patient cases, so I don't want to alarm you. Still, since drowsiness and breathing matter here, it's worth a quick chat with your pharmacist or doctor before combining them.
