Here's the wrinkle with co-codamol: the codeine part is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves your pain. Lab studies hint that boswellia may slow down that same enzyme. If that holds true in people, your body might convert less codeine into morphine, so you could notice weaker pain relief rather than more side effects.

I want to be honest that this is only a theory based on test-tube research, not real patients, so it may not amount to much in everyday use. The paracetamol half isn't affected this way. If you take co-codamol regularly and you're thinking about boswellia, just run it by your pharmacist or doctor first.

Here's the honest picture with co-codamol (codeine plus paracetamol) and boswellia. The interaction on file is based only on lab-dish research showing boswellic acids may slow a liver enzyme called CYP1A2. That's a long way from proven in real people, so I wouldn't lose sleep over it.

One thing worth knowing: codeine is a prodrug. Your body has to convert it (mainly through a different enzyme, CYP2D6) into morphine to relieve pain, so CYP1A2 isn't its main pathway. Practically, any effect from boswellia here is likely small and theoretical. Still, if you notice extra drowsiness, constipation, or feeling foggy, or if your pain relief changes, check in with your pharmacist or doctor before adjusting anything.
