Lomitapide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the thing about lomitapide: your liver clears it using an enzyme called CYP3A4, and the drug already carries a serious warning for liver problems and stomach side effects like nausea, diarrhea, and belly pain. Lab studies suggest Boswellia serrata can block that same CYP3A4 enzyme (and at low levels might even speed it up a little). If it blocks it, lomitapide could build up in your body and those liver and digestive effects could get worse.

To be fair, this is theoretical and comes only from test-tube research, not real patients, so we don't truly know how it plays out. But lomitapide is a high-stakes medicine, so I'd run any Boswellia use past your doctor or pharmacist before combining them.

About Lomitapide

What Is Lomitapide Used For?

Brand name: Juxtapid

Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.

Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
