Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase the levels of CYP2C9 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2C9 enzymes.

Opsynvi combines two medicines, macitentan and tadalafil, used together for pulmonary arterial hypertension. Both are partly cleared by a liver enzyme called CYP2C9. In test-tube studies, boswellic acids (the active part of Boswellia) appear to slow that enzyme down. In theory, that could let these drugs build up a bit higher than usual, which may nudge up their blood-pressure-lowering effects and side effects like headache, flushing, dizziness, an upset stomach, or feeling lightheaded when you stand up.

I want to be honest with you though: this is only a theoretical concern based on lab work, not real patient reports, so it may not amount to much in practice. Still, since Opsynvi already affects your blood pressure, it is worth a quick chat with your pharmacist or doctor before adding Boswellia. Please don't change your dose on your own.

Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Opsynvi combines two active medicines, macitentan and tadalafil, and both are processed by a liver enzyme called CYP3A4. Boswellia has been shown in test-tube studies to block this enzyme, which could theoretically slow how you clear these drugs and let their levels drift higher. If that happened, you might notice more of tadalafil's typical effects, like headache, flushing, a stuffy nose, indigestion, or lightheadedness, especially when standing up quickly.

I want to be honest though: this is a theoretical concern based only on lab research, not real patients, and boswellia has even nudged the enzyme the other way at low levels. So the direction isn't certain. Since Opsynvi treats a serious condition (pulmonary hypertension), please run the boswellia idea by your pharmacist or doctor first.
