Mercaptopurine falls under IMMUNOSUPPRESSANTS — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.

Here's the honest picture with this pairing. Mercaptopurine is a serious chemotherapy medicine that quiets down white blood cells and your immune system. Boswellia, the resin extract often taken for joint pain, seems to nudge the immune system too, though lab studies pull in different directions. Some suggest it calms immune activity, while other studies hint it can stimulate certain immune pathways. Because of that mixed picture, it could theoretically push mercaptopurine's effects in either direction.

This concern comes only from test-tube and animal research, not from people, so it's quite uncertain. Still, with a drug this powerful, I'd play it safe. Please run Boswellia by your oncology team or pharmacist before combining the two.

Mercaptopurine — available as a tablet and as an oral suspension — is used to treat acute lymphoblastic leukemia (ALL), a type of blood cancer. It is given as part of a combination chemotherapy maintenance regimen, meaning it works alongside other cancer medicines to keep leukemia in remission.

Both the tablet form and the oral suspension form carry this indication for adult and pediatric patients. The oral suspension is particularly useful for patients who cannot swallow tablets.
