Paricalcitol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Paricalcitol is a form of active vitamin D used to manage high parathyroid hormone levels in people with kidney disease. Your liver clears it partly through an enzyme called CYP3A4. Boswellia, in lab studies, seems to mostly slow this enzyme down (and at low amounts may slightly speed it up). If it slows clearance, more paricalcitol could linger in your system, which in theory could nudge your calcium too high. That can show up as nausea, constipation, a dry mouth, unusual thirst, or just feeling tired and foggy.

Honestly, this is a theoretical concern from test-tube research, not real patient cases, so I wouldn't lose sleep over it. Still, since calcium levels need careful watching here, let your pharmacist or doctor know before adding Boswellia so they can keep an eye on your bloodwork.
