Paricalcitol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for paricalcitol, the active vitamin D medicine often used in people with kidney disease to keep parathyroid hormone in check. Your liver clears this drug partly through an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could clear paricalcitol a little faster and leave you with less of it working than expected.

What that might look like in practice is paricalcitol not controlling your parathyroid levels as well as it should. I'll be honest though: this is a theoretical concern based on how the drugs work, not real-world reports, so I don't want to alarm you. Adrafinil itself carries its own risks, so please run this combination by your doctor or pharmacist first.
