Here's the gist of it. Your bladder medicine, tolterodine, gets broken down by a liver enzyme called CYP2D6. Boswellia, at least in test-tube studies, seems to slow that enzyme down a bit. If that happened in your body, tolterodine could clear out more slowly and build up, which can nudge its usual side effects upward. Think dry mouth, constipation, blurry vision, dizziness, or a faster heartbeat.

Now, the honest part: this is only a theoretical concern based on lab work, not real patient cases, so it may not amount to much in everyday life. Still, if you plan to use a boswellia supplement regularly, give your pharmacist or doctor a heads-up so they can keep an eye on how you're feeling.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the situation with tolterodine (Detrol) and boswellia. Your body relies on a liver enzyme called CYP3A4 to help break tolterodine down. In lab dish studies, boswellic acids from the resin seem to slow that enzyme down (and at very low amounts may slightly speed it up). If the enzyme is slowed, tolterodine could linger a bit longer and build up, which might mean stronger anticholinergic effects like dry mouth, constipation, blurry vision, or feeling dizzy or drowsy.

I want to be honest though: this is theoretical, based only on test-tube work, not real patients. For most people it likely won't cause trouble, but if you notice those side effects ramping up, give your pharmacist or doctor a quick call before changing anything.

It targets three main symptoms: a sudden, hard-to-control urge to urinate (urgency), urinating too often (frequency), and leaking urine when that urge hits (urge urinary incontinence).
