Why this applies to Ondansetron
Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Broccoli interacts with.
Pharmacokinetic research in humans shows that eating 500 grams of fresh broccoli daily for 6-12 days can increase CYP1A2 activity by 10% to 200%. Induction of CYP1A2 activity by broccoli is attributed to its glucosinolate constituents.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: ondansetron (Zofran) is partly broken down by a liver enzyme called CYP1A2. Eating large amounts of broccoli may speed that enzyme up a bit, which could clear ondansetron from your body a little faster. In theory, that might make it work less well, so you could still feel some nausea or vomiting even after taking your dose. Ondansetron is the active drug itself, not a prodrug, so faster breakdown means weaker effect, not stronger.
Keep this in perspective. The research used about 500 grams of fresh broccoli daily for many days, far more than a normal serving, and the evidence is fairly weak. Enjoying broccoli at meals is unlikely to be a problem. If you rely on Zofran during chemo and notice it working less well, check in with your pharmacist or doctor.

What Is Ondansetron Used For?

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.
Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
