Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with Prezcobix (darunavir plus cobicistat). Both of these rely on the CYP3A4 enzyme. Cobicistat is actually in there to block CYP3A4 and keep darunavir levels high enough to fight HIV. In lab dishes, Bulbine natalensis appears to do the opposite, revving up CYP3A4. If that happened in your body, it could speed up clearance of darunavir and pull its blood levels down.

Why that matters: with an HIV medication, lower levels can mean the virus isn't fully controlled, which risks treatment failure and resistance over time. I want to be honest though, this is only test-tube data, not human studies, so it's theoretical. Still, because keeping HIV suppressed is so important, I'd steer clear and check with your doctor or pharmacist before adding this supplement.

What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.

Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
