Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the tricky part with irinotecan (Camptosar): it's actually a prodrug. Your body has to convert it into its active form, called SN-38, which is the part that fights the cancer. The enzyme CYP3A4 is involved in handling irinotecan, and lab studies suggest Bulbine natalensis may speed up (induce) that enzyme.

In theory, revving up this enzyme could shift how much active drug your body produces and shorten how long the medicine stays around, which matters a lot for a chemotherapy with such a narrow safety window. Honestly though, this is based only on lab and animal data, not real people, so the true effect in humans is unknown. With a cancer drug this serious, please don't add Bulbine natalensis without first clearing it with your oncologist or pharmacist.
