Nevirapine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing Bulbine natalensis and your HIV medicine, nevirapine. Your body clears nevirapine using a liver enzyme called CYP3A4. In lab studies, Bulbine appears to rev up that enzyme. If that happened in people, it could speed up how fast nevirapine is broken down, leaving less of it in your blood.

Why that matters: with HIV treatment, steady drug levels keep the virus suppressed. Lower levels could mean the medicine works less well and, over time, the virus could become resistant. That's a big deal. The good news is this is only theoretical and based on lab work, not human studies, so don't panic. But please don't add Bulbine without first checking with your HIV doctor or pharmacist.

About Nevirapine

What Is Nevirapine Used For?

Brand names: Viramune, Viramune XR

Nevirapine is used alongside other antiretroviral medicines to treat HIV-1 infection. It is not a cure, but it helps suppress the virus when used as part of a combination regimen.

The immediate-release tablet is approved for adults and pediatric patients 15 days and older. The extended-release tablet is approved for adults and for pediatric patients 6 years of age or older who meet a minimum body surface area requirement. The oral suspension is approved for adults and pediatric patients 15 days and older. None of these formulations should be used alone to treat HIV-1, and none are approved for post-exposure prophylaxis (PEP).
