Why this applies to Paclitaxel

Paclitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing this supplement with paclitaxel. Your body clears paclitaxel partly through a liver enzyme called CYP3A4. In lab dishes, Bulbine natalensis appears to speed that enzyme up. If that held true in people, your body might break down paclitaxel faster, leaving less of it to fight the cancer. Paclitaxel is not a prodrug, so faster clearance simply means a weaker effect, and with chemotherapy you really don't want to risk underdosing.

That said, this is only test-tube evidence and has never been confirmed in humans. On top of that, animal studies have flagged possible liver and kidney harm with this supplement. Please don't add it during chemo without first running it by your oncologist or pharmacist.

Here's the situation with Bulbine natalensis and your paclitaxel (Taxol, Abraxane). Paclitaxel is moved around and out of cells partly by a transporter pump called P-glycoprotein. Our file note says this is purely theoretical and based on lab-dish research, not human studies. In that lab work, the plant seemed to turn up the gene for this pump. If that actually happened in the body, more of the pump could push paclitaxel out faster, which might lower how much medicine reaches the cancer and weaken the treatment.

I want to be honest: this is very weak, indirect evidence, and the real effect in people is unknown. But since paclitaxel is chemotherapy where getting the right amount truly matters, please don't add this supplement without first checking with your oncology team or pharmacist.

About Paclitaxel (with albumin) Injection

What Is Paclitaxel (with albumin) Injection Used For?

Brand names: Taxol, Abraxane

Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound) is used to treat three types of cancer. It treats metastatic breast cancer in patients whose prior chemotherapy has stopped working or who relapsed within 6 months of completing adjuvant (post-surgery) chemotherapy. It is also used as a first-line treatment for locally advanced or metastatic non-small cell lung cancer (NSCLC) — given together with carboplatin — in patients who cannot have surgery or radiation therapy.

Additionally, it is approved as a first-line treatment for metastatic adenocarcinoma of the pancreas, given in combination with gemcitabine. All three uses apply specifically to Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound), including the brands Abraxane and related albumin-bound formulations.
