Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for remdesivir (Veklury). Lab studies suggest Bulbine natalensis can speed up an enzyme called CYP3A4, which is one of the pathways that processes remdesivir. In theory, revving up that enzyme could clear the drug faster and make this antiviral less effective at stopping COVID-19 from spreading in the body. So the direction here is reduced effect, not extra side effects.

That said, I want to keep this in perspective. This is only test-tube evidence, never confirmed in people, and remdesivir is given as a short IV course in a supervised hospital or clinic setting. Still, this is exactly the kind of thing worth mentioning to your doctor or pharmacist before mixing the two.

Here's the situation with Bulbine natalensis and remdesivir (Veklury). Remdesivir is moved around the body partly by a transporter called P-glycoprotein. Lab (test tube) studies hint that Bulbine may increase the activity of this transporter, which in theory could pump remdesivir out faster and lower its levels. Since remdesivir works by stopping the virus from copying itself, weaker levels could mean a less effective treatment.

I want to be honest, though: this is purely theoretical and comes only from cell studies, not from people. On top of that, remdesivir is given by IV in a medical setting, where your team monitors you closely. Because Bulbine itself carries liver and kidney concerns, please run it by your doctor or pharmacist first.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
