Repotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern in plain terms. Repotrectinib (Augtyro), the targeted cancer medicine used for certain ROS1 or NTRK-positive tumors, is cleared from your body mainly by a liver enzyme called CYP3A4. Lab studies suggest Bulbine natalensis may speed up that enzyme. If that holds true in people, your body could break down repotrectinib faster, leaving less of it in your system. For a cancer drug, that matters, because lower levels could mean it doesn't fight the tumor as well as it should.

I want to be honest, though: this is only theoretical, based on test-tube research, not real patients. Even so, given the stakes with a cancer treatment, please loop in your oncologist or pharmacist before adding this supplement.

Here's the short version on mixing Bulbine natalensis with Repotrectinib (Augtyro), a targeted cancer medicine used for certain ROS1 and NTRK-positive tumors. Repotrectinib is carried around your body partly by a transporter called P-glycoprotein. The concern on file is theoretical and comes only from lab (test tube) research, not from people, so please don't panic over it.

One database note says this plant might raise repotrectinib levels, but the actual lab data suggests the plant may speed up that transporter, which could instead lower how much drug stays in your system and weaken its cancer-fighting effect. Because the evidence points in conflicting directions and is very weak, the honest answer is we don't know. On top of that, this supplement has its own liver and kidney safety questions. Please check with your oncology pharmacist or doctor before combining them.
