Why this applies to Upadacitinib Hemihydrate
Upadacitinib Hemihydrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking with Rinvoq (upadacitinib). Your body relies partly on an enzyme called CYP3A4 to break it down. In lab dishes, Bulbine natalensis seemed to speed up that enzyme. If that also happened in people, it could clear upadacitinib out faster and leave less of it working in your system. The concern there isn't extra side effects, it's the opposite: your arthritis or eczema control could slip, meaning more joint pain, swelling, stiffness, or itchy, flaring skin.
I want to be honest that this is theoretical. It comes from test-tube work, not from real patients, so we can't say how big the effect would be, if any. Still, since Rinvoq is doing important work for you, please check with your pharmacist or doctor before adding this supplement.

What Is Upadacitinib Used For?
Brand name: Rinvoq
Upadacitinib (sold as Rinvoq extended-release tablets and Rinvoq LQ oral solution) treats several inflammatory conditions. Most uses are for people who did not respond well to, or could not tolerate, TNF blockers (another type of anti-inflammatory medicine).
Not every product is approved for every use. Rinvoq LQ oral solution is labeled only for psoriatic arthritis and polyarticular juvenile idiopathic arthritis. Rinvoq is labeled for all the uses listed in the guide below.
