Here is the interaction we found between these two, with the evidence behind it.
CNS DEPRESSANTS
Why this applies to Tramadol, Acetaminophen
Tramadol, Acetaminophen falls under CNS DEPRESSANTS — the group California Poppy interacts with.
Laboratory research shows that California poppy has sedative effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Tramacet combines tramadol (an opioid pain reliever) with acetaminophen. Tramadol on its own can make you feel drowsy, foggy, or lightheaded. California poppy is a calming herb that, at least in lab and animal studies, seems to have its own mild sedative effect. Stack the two together and you may feel sleepier or more sluggish than you expect, with possible dizziness or slowed thinking.
I want to be honest about the evidence here. This concern comes from test-tube and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, because tramadol affects the brain, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you drive.

From our pharmacist’s overview on the Tramadol, Acetaminophen page
“The combinations I worry about most with tramadol and acetaminophen (Tramacet) involve anything that deepens drowsiness or strains the liver. Tramadol is an opioid, so sedating supplements like kava, along with alcohol in any form, can slow breathing on top of adding sleepiness, which is why this list deserves genuine care rather than…”
