Cauliflower has a moderate interaction with Sofosbuvir, Velpatasvir, Voxilaprevir. Preliminary clinical evidence suggests that eating cruciferous vegetables, including broccoli, cauliflower, daikon radish sprouts, and cabbage, can increase cytochrome P450 1A2 (CYP1A2) activity by 14% to 27%.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Sofosbuvir, Velpatasvir, Voxilaprevir
Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cauliflower interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the good news first: I wouldn't lose sleep over eating cauliflower while you're on Vosevi. The interaction on file says cruciferous veggies can nudge a liver enzyme called CYP1A2 to work a bit faster. But Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) isn't really cleared by CYP1A2 in any meaningful way, so this particular concern doesn't map well onto your hepatitis C medicine.
On top of that, the evidence here is weak and the effect is small, and normal food amounts of cauliflower are very different from concentrated supplement pills. To stay safe and keep your treatment working its best, just run your supplement plan past your pharmacist or doctor before adding anything new.

From our pharmacist’s overview on the Sofosbuvir, Velpatasvir, Voxilaprevir page
“Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) cures hepatitis C over a single short course, and that is exactly why I would be genuinely careful with supplements while taking it. Anything that drags its blood levels down can cost you the cure itself.”

About Cauliflower
What is Cauliflower?

- Part of a heart-healthy diet
Cauliflower is a nutritious cruciferous vegetable that provides vitamin C, fiber, and plant compounds called glucosinolates. As a food it is healthy and safe for most people, but there is no strong proof that cauliflower supplements treat or prevent any specific disease.
