Ceylon Cinnamon and Diltiazem

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Diltiazem

Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Ceylon Cinnamon interacts with.

Theoretically, Ceylon cinnamon might have additive effects with antihypertensive drugs and increase the risk of hypotension.
Animal research shows that Ceylon cinnamon extract has vasorelaxant properties and reduces blood pressure in rat models of hypertension, possibly via inhibition of calcium influx through L-type voltage-sensitive channels.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: diltiazem (Cardizem and its other versions) already lowers your blood pressure by relaxing your blood vessels, partly by slowing calcium from entering muscle cells. Interestingly, lab and animal studies suggest Ceylon cinnamon may work in a somewhat similar way and gently lower blood pressure too. So the worry is simply additive: stacked together they could nudge your pressure a little lower than diltiazem alone.

I want to be honest about the evidence, though. This is theoretical and based only on animal research, so it's far from a sure thing. A sprinkle of cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon capsules are the bigger question. If you start them, watch for dizziness, lightheadedness, or feeling faint, and check in with your pharmacist or doctor first.

From our pharmacist’s overview on the Diltiazem page

“Someone on diltiazem (Cardizem, Tiazac) is usually managing blood pressure, angina, or a fast heart rhythm, so this list deserves genuine care. Seven of the 260 ingredients on file carry a major rating, and two of those are things people actually buy: grapefruit, which blocks the enzyme that clears diltiazem and can push its blood leve…”

About Ceylon Cinnamon

What is Ceylon Cinnamon?

Ceylon Cinnamon
  • Blood sugar support
  • High cholesterol
  • Indigestion and gas
  • Antimicrobial
  • General flavoring and culinary use

Ceylon cinnamon is the so-called 'true' cinnamon, valued as a spice and used in traditional medicine for blood sugar, cholesterol, and digestion. Evidence for most health benefits is limited and mixed, but as a food it is generally safe and contains far less coumarin than common cassia cinnamon. Talk with your pharmacist or doctor before taking concentrated supplements, especially if you have a health condition or take medications.

About Diltiazem

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
