Here's the simple version: treprostinil (Orenitram) already works to widen your blood vessels and lower pressure in your lungs, and one common side effect is that it can drop your overall blood pressure too. Cod liver oil, thanks to its omega-3s, can also nudge blood pressure down a little. Put them together and those effects may add up.

What that could feel like is more lightheadedness, dizziness, headache, or flushing, especially when you stand up quickly. The good news is this concern is mostly theoretical and based on modest evidence, so for many people it won't be a big deal. Still, since treprostinil is potent, it's worth a quick chat with your pharmacist or doctor before adding cod liver oil, rather than starting or stopping anything on your own.

Here's the connection worth knowing. Treprostinil (Orenitram) works partly by relaxing blood vessels and keeping platelets from clumping together, so it already nudges your blood toward bleeding a little more easily. Cod liver oil's omega-3s (EPA and DHA) can do something similar, making platelets less sticky and lengthening bleeding time. Stack the two and that effect could add up.

To be fair, this is mostly theoretical, and the bleeding seen in studies came from large doses (about 20 to 40 mL a day), not a small daily spoonful. Still, watch for easy bruising, nosebleeds, bleeding gums, or cuts that are slow to stop. Before adding cod liver oil, run it by your pharmacist or doctor first.

Evidence is strongest for its nutrient content, but proof of specific disease benefits is mixed. About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
