Cod Liver Oil and Iloprost

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Cod Liver Oil interacts with whole categories of medication. Iloprost falls into 2 of those categories, so it can interact with Cod Liver Oil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Iloprost

Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Cod Liver Oil interacts with.

Theoretically, taking cod liver oil with antihypertensive drugs might increase the risk of hypotension.
Cod liver oil can lower blood pressure and might have additive effects in patients treated with antihypertensives; use with caution.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: iloprost (Ventavis) is an inhaled medicine that widens your blood vessels to ease the pressure in your lungs. One of its known effects is lowering blood pressure overall. The omega-3 fats in cod liver oil can also nudge blood pressure down a little, so stacking the two together could add up and leave you more prone to low blood pressure. That might show up as feeling lightheaded, dizzy, faint, or unusually tired, especially when you stand up quickly.

This concern is mostly theoretical and the supporting research is fairly modest, so for many people the effect is small. Still, since iloprost already affects blood pressure, it's worth mentioning your cod liver oil to your pharmacist or doctor before combining them, rather than changing anything on your own.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Iloprost also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, cod liver oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
High doses of cod liver oil, 20-40 mL daily, have been associated with prolonged bleeding time and reduced platelet aggregation in healthy volunteers.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing cod liver oil with Iloprost (Ventavis). Iloprost doesn't just relax blood vessels in your lungs, it also makes your platelets a little less sticky, which can nudge your bleeding risk up. Cod liver oil, especially in big doses (think 20 to 40 mL a day), can do something similar by slowing how well platelets clump together. Stack the two and you may bruise more easily, have nosebleeds, or notice bleeding that takes longer to stop.

This is more theoretical than proven, and small food or everyday supplement amounts likely won't cause trouble. Still, please run it by your pharmacist or doctor before adding cod liver oil, so they can check your dose and keep an eye out.

  • Joint and bone health
  • Reducing inflammation

Cod liver oil supplies omega-3 fatty acids (EPA and DHA) along with vitamins A and D, and is popular for heart, joint, and general health support.
