Cod Liver Oil and Diltiazem

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Diltiazem

Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Cod Liver Oil interacts with.

Theoretically, taking cod liver oil with antihypertensive drugs might increase the risk of hypotension.
Cod liver oil can lower blood pressure and might have additive effects in patients treated with antihypertensives; use with caution.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: cod liver oil, thanks to its omega-3s, can nudge your blood pressure down a little on its own. Diltiazem already lowers blood pressure by relaxing your blood vessels, so stacking the two together could add up and push your pressure lower than intended.

In real life that might show up as feeling dizzy or lightheaded, especially when you stand up quickly, plus tiredness or a flushed feeling. It's worth knowing this is mostly a theoretical concern from lower-quality studies, and ordinary food amounts of fish are unlikely to matter much. Still, since you're on a blood pressure medicine, it's smart to check with your pharmacist or doctor and keep an eye on your numbers before adding a concentrated supplement.

“Someone on diltiazem (Cardizem, Tiazac) is usually managing blood pressure, angina, or a fast heart rhythm, so this list deserves genuine care. Seven of the 260 ingredients on file carry a major rating, and two of those are things people actually buy: grapefruit, which blocks the enzyme that clears diltiazem and can push its blood leve…”

About Cod Liver Oil

What is Cod Liver Oil?

Cod Liver Oil
  • Source of omega-3 fatty acids
  • Vitamin A and D supplementation
  • Heart health
  • Joint and bone health
  • Reducing inflammation

Cod liver oil supplies omega-3 fatty acids (EPA and DHA) along with vitamins A and D, and is popular for heart, joint, and general health support. Evidence is strongest for its nutrient content, but proof of specific disease benefits is mixed. Because it contains vitamin A, taking too much can be harmful, so check with your pharmacist or doctor before using it.

About Diltiazem

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
