Here's the simple version: coffee charcoal works like a sponge in your gut, with a porous surface that can grab onto things passing through, including your blood pressure pill. If you take your lisinopril/hydrochlorothiazide at the same time as coffee charcoal, less of the medicine may actually get absorbed into your body. That could mean it doesn't lower your blood pressure as well as it should.

The good news is this is easy to manage. Just space them apart, taking your medication and the coffee charcoal at least two hours apart. I'll be honest that this concern is based on how charcoal behaves in theory, not on strong studies, but separating the timing is a smart, no-cost habit. If your blood pressure has been creeping up, check in with your pharmacist or doctor.

Lisinopril and hydrochlorothiazide tablets — sold as the brand Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events such as stroke and heart attack.

This is a fixed-dose combination product and is not intended as a first-line (initial) treatment for high blood pressure. It is typically prescribed for patients whose blood pressure is not adequately controlled on a single medication alone.
