Here's the simple version: coffee charcoal acts a lot like other types of charcoal in your gut. Its porous surface can grab onto things passing through, and that includes your antibiotic. If you swallow Pivmecillinam (Selexid) too close to a dose of coffee charcoal, the charcoal may bind some of the drug before your body absorbs it, so less actually gets into your system.

Why that matters: Pivmecillinam is treating a urinary tract infection, and it needs to reach high enough levels to clear the bacteria. Soaking up part of the dose could mean a weaker effect and a higher chance the infection isn't fully knocked out. This concern is based on how charcoal works, not on direct studies, so think of it as a sensible precaution. The easy fix is timing: keep them at least two hours apart, and check with your pharmacist if you're unsure.
