Sitagliptin, Metformin falls under ANTIDIABETES DRUGS — the group Colocynth interacts with.

Theoretically, colocynth might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that colocynth fruit powder reduces fasting blood glucose levels in patients with type 2 diabetes who are also taking antidiabetes drugs.

Here's the thing to keep in mind if you're on Zituvimet (sitagliptin plus metformin) and thinking about colocynth. This gourd already has its own blood-sugar-lowering effect. A few small studies found that colocynth fruit powder pushed fasting blood sugar down in people with type 2 diabetes who were also on their regular diabetes medicine. Stack that on top of what your medication is already doing, and your sugar could dip too low.

Low blood sugar can look like shakiness, sweating, dizziness, confusion, or a racing heart. Honestly though, my bigger worry is that colocynth is genuinely toxic and can cause severe, dangerous poisoning even in small amounts, so I'd steer clear of it entirely. Please talk with your doctor or pharmacist before trying anything like this.
