Nafcillin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Comfrey interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid constituents in comfrey.
CYP3A4 enzymes convert pyrrolizidine alkaloids, constituents of comfrey, to toxic metabolites. Some case reports show that enzyme inducers, such as phenobarbital, seem to enhance the toxicity of comfrey.

Here's the key thing to understand about this combination: it's not that comfrey changes how your nafcillin works. It's the other way around. Nafcillin is a penicillin antibiotic that can speed up a liver enzyme called CYP3A4. That same enzyme is what turns the natural compounds in comfrey (called pyrrolizidine alkaloids) into toxic byproducts that can harm your liver.

So in theory, if you're getting nafcillin and also using comfrey, your liver might churn out more of those harmful breakdown products. This is a theoretical concern based on how the chemistry works, not something proven for nafcillin specifically. Still, comfrey already carries real liver risks and should never be swallowed. Please check with your pharmacist or doctor before using any comfrey product while on this antibiotic.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

What Is Nafcillin Injection Used For?

Brand name: Nallpen

NAFCILLIN Injection is used to treat infections caused by staphylococci — specifically strains that produce an enzyme called penicillinase, which makes them resistant to regular penicillin. Susceptibility testing is performed to confirm the bacteria will respond to nafcillin before or shortly after starting treatment.

Nafcillin may also be started right away when a resistant staph infection is strongly suspected, even before test results are available. It should not be used for infections caused by bacteria that would respond to standard penicillin G, or for infections caused by non-staph organisms.
