Why this applies to Flutamide

Flutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Here's the gist for you. Flutamide is processed by a liver enzyme called CYP3A4. Cranberry, at least in theory, may slow that enzyme down a little. If that happens, more flutamide could linger in your body than expected, which might nudge up the chance of side effects like nausea, diarrhea, or changes in your liver function tests. Flutamide already needs liver monitoring, so that's the part worth keeping an eye on.

That said, the evidence here is thin. It rests mostly on a single case report with a different drug plus some animal data, not solid studies with flutamide itself. An occasional glass of cranberry juice is unlikely to be a problem, but daily concentrated cranberry supplements are worth mentioning to your oncology team or pharmacist first.

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
