Why this applies to Brexpiprazole

Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the connection worth knowing: your body relies on a liver enzyme called CYP3A4 to help break down brexpiprazole (Rexulti). In lab tests, certain compounds from cubeb berries have slowed that same enzyme down. If that happened in people, brexpiprazole could clear more slowly and build up a bit, which might mean stronger effects and more side effects like drowsiness, dizziness, restlessness, or weight changes.

The good news is that this is only theoretical right now, based on test-tube studies, not real cases in humans. A sprinkle of cubeb as a kitchen spice almost certainly won't matter. Concentrated extracts or oils are a different story, so if you're considering those, please run it by your pharmacist or doctor first. Don't change your Rexulti on your own.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
