Why this applies to Darunavir, Ritonavir

Darunavir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the short version: some compounds in cubeb berries have slowed down an enzyme called CYP3A4 in lab dishes. Both darunavir and ritonavir lean on this same enzyme. In fact, ritonavir is often added on purpose as a 'booster' because it blocks CYP3A4 to keep darunavir levels up. If cubebs added their own blocking effect, in theory your levels could creep higher, which might mean more nausea, diarrhea, headache, or stomach upset.

The reassuring part: this has only been seen in test-tube studies, never in real people, so the concern is theoretical. A little cubeb as a kitchen spice is very unlikely to matter. Concentrated extracts or oils are a different story. Since these HIV medicines are ones you don't want fluctuating, please run any cubeb supplement past your pharmacist or doctor first.
