Why this applies to Gilteritinib Fumarate

Gilteritinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Your leukemia medication, gilteritinib (Xospata), is cleared from your body largely by a liver enzyme called CYP3A4. Some compounds in cubeb berries have been shown, in test tubes only, to slow that same enzyme down. If that were to happen in people, gilteritinib could build up more than expected, which might mean stronger side effects like nausea, fatigue, muscle or joint pain, swelling, or changes to your heart rhythm (QT prolongation).

The important part: this is theory based on lab studies, not something seen in actual patients. The small amount of cubeb you would get as a kitchen spice is very unlikely to matter. Concentrated extracts or oils are the bigger question mark. Since gilteritinib is a serious cancer drug, please run any cubeb supplement past your oncologist or pharmacist first.

What Is Gilteritinib Used For?

Brand name: Xospata

Xospata (gilteritinib) is used to treat adults with acute myeloid leukemia (AML) — a fast-moving blood and bone marrow cancer — when the disease has come back after prior treatment (relapsed) or stopped responding to treatment (refractory). It is only used when a laboratory test approved by the FDA has confirmed that the leukemia cells carry a FLT3 mutation, a genetic change that helps the cancer grow.

Because response can take time, treatment for at least 6 months is generally recommended as long as the disease is not progressing and side effects are manageable.
