Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Your body relies on a liver enzyme called CYP3A4 to break down nelfinavir (Viracept) and keep it at a steady level. Lab studies suggest a few compounds in cubeb berries can slow that same enzyme. In theory, if cubebs block CYP3A4, nelfinavir could clear out of your system more slowly and build up a bit higher than usual. That could mean a greater chance of side effects like diarrhea, nausea, gas, or stomach upset, which are already common with this drug.

I want to be honest: this is only seen in test tubes so far, never reported in real people, so the risk is mostly theoretical. A little cubeb used as a kitchen spice is unlikely to matter. Concentrated extracts or oils are a different story, so check with your pharmacist or doctor before using them.
