Why this applies to Sunitinib

Sunitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the concern with pairing diosmin and sunitinib. Your body relies on a liver enzyme called CYP3A4 to clear sunitinib out of your system. Diosmin may slow that same enzyme down, so in theory sunitinib could build up to higher levels than expected. With a cancer drug like this, higher levels can mean more of the side effects you already watch for, such as fatigue, mouth sores, high blood pressure, diarrhea, hand-and-foot skin reactions, or changes in blood counts.

I want to be honest that this is mostly theoretical. The clearest human data is with carbamazepine, not sunitinib, and lab studies on diosmin and CYP3A4 actually disagree. Still, because sunitinib is a serious medication with a narrow safety margin, please run any diosmin product by your oncologist or pharmacist first.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Sunitinib Used For?

Brand name: Sutent

Sunitinib malate capsules and Sutent capsules are both approved to treat adult patients with gastrointestinal stromal tumor (GIST) after the cancer has progressed on or could not be tolerated with imatinib mesylate. Both are also used for advanced renal cell carcinoma (kidney cancer) and as adjuvant (preventive) treatment after surgery for adults at high risk of kidney cancer coming back. Additionally, both treat progressive, well-differentiated pancreatic neuroendocrine tumors (pNET) that cannot be surgically removed or have spread.
