Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Eleuthero interacts with.

Theoretically, eleuthero may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that a constituent of eleuthero, dihydroxybenzoic acid, appears to inhibit platelet aggregation. Concomitant use with anticoagulant or antiplatelet drugs might increase the risk of bleeding. This effect has not been reported in humans.

Here's the short version: alteplase (tPA) is a powerful clot-busting drug given in the hospital for things like a stroke, heart attack, or major blood clot. It works fast and already carries a real risk of bleeding on its own. Eleuthero contains a compound that, in lab and animal studies, seems to make platelets a little less sticky. On paper, stacking that on top of a clot-buster could nudge the bleeding risk higher.

I want to be honest about the evidence here, though. This is theoretical and has not actually been seen in people, so I don't want to alarm you. Practically speaking, alteplase is an emergency medication your care team controls, not something you mix at home. Just be sure the doctors and pharmacist know about any eleuthero you take so they can factor it in.
