Aspirin, Caffeine, Orphenadrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eleuthero interacts with.
In vitro and animal research suggest that standardized extracts of eleuthero inhibit CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill has three parts, and the one to watch with eleuthero is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2. In lab and animal studies, eleuthero appears to slow that enzyme down, which could let caffeine linger longer than usual. In plain terms, you might feel more of caffeine's effects: jitteriness, a racing heart, trouble sleeping, or feeling wired. The aspirin and orphenadrine portions are not really affected here.
Here's the honest part: this has only been seen in test tubes and animals, never confirmed in people, so the real-world risk looks small. Still, if you notice extra restlessness or a pounding heart, mention it to your pharmacist or doctor before changing anything.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro and animal research shows that a constituent of eleuthero, dihydroxybenzoic acid, appears to inhibit platelet aggregation. Concomitant use with anticoagulant or antiplatelet drugs might increase the risk of bleeding. This effect has not been reported in humans.

Here's the gist with this combo product: the part that matters for eleuthero is the aspirin. Aspirin already thins the blood by keeping platelets from clumping together. Lab and animal studies hint that eleuthero may do a little of the same thing, so stacking them could nudge your bleeding risk up a bit. That might look like easier bruising, longer bleeding from small cuts, nosebleeds, or stomach upset.
I want to be honest about the strength here: this is only theoretical, based on test-tube and animal data, and it has never been reported in actual people. So it's worth a mention, not a panic. If you take this regularly or have any bleeding concerns, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
This combination pill blends aspirin, caffeine, and orphenadrine (a muscle relaxer). The concern on file is that eleuthero might slow down a liver enzyme called CYP3A4, which helps break down the orphenadrine part. In theory, that could let orphenadrine build up a little, nudging up side effects like drowsiness, dry mouth, blurry vision, or a faster heartbeat.
Here's the reassuring part: this idea comes mostly from lab and animal studies. When researchers actually gave healthy people eleuthero for two weeks, it did not meaningfully block CYP3A4. So the real-world risk looks low. Still, since this is a sedating medicine, mention any new herbal supplement to your pharmacist or doctor before mixing them.
