Why this applies to Diltiazem, Enalapril

Diltiazem, Enalapril falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Your medication Teczem combines diltiazem (a calcium channel blocker for blood pressure and heart rate) with enalapril (an ACE inhibitor). The piece that matters here is the diltiazem, which is moved around the body partly by a transporter called P-glycoprotein. In lab dish studies, eleuthero seems to slow that transporter down, which in theory could let a bit more diltiazem build up in your system.

If that happened, you might notice more of diltiazem's usual effects, like lower blood pressure, dizziness, a slower heartbeat, or swelling in the ankles. That said, this has only been seen in test tubes, never reported in real people, so I wouldn't lose sleep over it. Still, since blood pressure meds are worth keeping steady, mention the eleuthero to your pharmacist or doctor before combining them.

Diltiazem, Enalapril also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

This combo pill pairs diltiazem (a calcium channel blocker) with enalapril (an ACE inhibitor) to lower blood pressure. The diltiazem part is broken down by a liver enzyme called CYP3A4. On paper, eleuthero might slow that enzyme down, which could let diltiazem build up a bit and lead to lower blood pressure, dizziness, a slower heart rate, or swelling in the ankles.

Here's the reassuring part: this concern comes mostly from lab and animal tests. When researchers gave eleuthero to healthy people for two weeks, it did not meaningfully change CYP3A4 activity. So a real-world problem is unlikely. Still, if you notice dizziness or feel washed out, give your pharmacist or doctor a quick call before changing anything.
