Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist with ivosidenib (Tibsovo) and eleuthero. Your body clears ivosidenib mainly through a liver enzyme called CYP3A4. In lab dishes and animal studies, eleuthero looked like it might slow that enzyme down a bit. If that were true in people, ivosidenib could build up and possibly raise the risk of its side effects, including a serious heart rhythm change (QT prolongation), nausea, fatigue, or a reaction called differentiation syndrome.

The reassuring part: when researchers actually gave eleuthero to healthy volunteers, it did not meaningfully affect CYP3A4. So this concern is mostly theoretical and rated unlikely. Still, ivosidenib is a serious cancer drug, so please run any supplement past your oncologist or pharmacist before adding it.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
