Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ergot interacts with.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the gist for this pair. Your body relies on an enzyme called CYP3A4 to break down ergot's active chemical, ergotamine. Ketoconazole is a strong blocker of that very enzyme. So if the two are taken together, ergot can't be cleared the usual way and may pile up in your system.

When ergot builds up, you can get too much of its blood-vessel-tightening effect. That can show up as cold, numb, or tingling fingers and toes, leg or muscle cramps, headache, or chest pain. Honestly though, raw ergot isn't something anyone should be taking, since it's toxic on its own. This warning rests on theoretical, anecdotal evidence, but the combination still isn't worth the risk. Please run it by your pharmacist or doctor first.

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
