CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS
Why this applies to Ketoconazole
Ketoconazole falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Caffeine interacts with.
Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here the medication is actually doing the affecting, not the other way around. Ketoconazole can slow down a liver enzyme called CYP1A2, which is one of the main pathways your body uses to clear caffeine. When that enzyme is partly blocked, caffeine may hang around longer than usual and build up a bit. In plain terms, your morning coffee or energy drink could hit harder and last longer than you expect.
That might show up as feeling jittery, a racing or pounding heart, trouble sleeping, or just feeling wired. Keep in mind this is a theoretical, minor interaction based only on lab data, so for most people it is mild. Still, if you notice extra caffeine jitters while on ketoconazole, ease back on caffeine and check with your pharmacist or doctor.

What Is Ketoconazole Used For?

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.
This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
