Why this applies to Rifampin
Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for rifampin. Lab work hints that the berberine in European barberry may gently slow down a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear rifampin. In theory, blocking that pathway could let a bit more rifampin linger, which might nudge up side effects like nausea, stomach upset, or liver irritation.
I'll be honest though, this is very early, theory-only stuff, not something proven in people, so I wouldn't lose sleep over it. The bigger wrinkle is that rifampin itself is a powerful enzyme booster and can change how many other drugs and supplements work. Please loop in your pharmacist or doctor before pairing barberry with it.

European barberry is a shrub whose root, bark, and berries contain berberine, a bitter plant alkaloid that has been studied mostly in isolated form. Some early research on berberine is promising for blood sugar and cholesterol, but high-quality evidence for the whole barberry herb is limited. What Is Rifampin Used For?
Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor
Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.
It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.
Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
