Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with alvimopan (Entereg) and European mandrake. Mandrake's tropane alkaloids slow down how fast your gut moves things along. When your bowel empties more slowly, a pill like alvimopan can sit in your system longer and get absorbed a bit more than expected, so its effects could be amplified. I want to be honest that this is a theoretical concern based on how these chemicals behave, not something proven in real patients.

The bigger problem here is mandrake itself. It is genuinely toxic and can cause a dangerously dry mouth, blurred vision, a racing heart, confusion, and worse. Please don't take it in any form. If you have questions about alvimopan after your surgery, talk with your pharmacist or surgical team first.
