First things first: please don't take European mandrake at all. It's a toxic plant with no proven benefit, and that's true whether or not you're on a cancer medicine. Setting that aside, here's the specific concern with erdafitinib (Balversa). Mandrake's tropane alkaloids slow down how fast your gut empties. When food and pills sit longer in the digestive tract, a swallowed medicine like erdafitinib can be absorbed more completely, which may nudge its blood levels higher.

For erdafitinib, more drug in your system can mean stronger side effects, things like mouth sores, dry or irritated eyes, high phosphate levels, nail and skin changes, and diarrhea. To be fair, this interaction is theoretical, based on how the chemicals behave rather than real cases. Still, please loop in your oncologist or pharmacist before adding anything.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
