First things first: European mandrake is genuinely toxic, and I would never recommend taking it. Putting that aside, here is the idea behind this interaction. Mandrake's tropane alkaloids (atropine-like compounds) slow down how fast your gut moves things along. When the gut slows, a pill like Levamisole may sit there longer, and your body could absorb a little more of it than usual.

For Levamisole, more drug in your system could mean stronger side effects, things like nausea, a metallic taste, dizziness, or flu-like aches. I'll be honest with you though: this is a theoretical concern based on how these substances work, not something proven in real patients. Please don't use mandrake, and check with your pharmacist or doctor about anything you take alongside Levamisole.
