Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

This combination cold product is a mix of four ingredients, and the one that overlaps with Evodia's known effects is acetaminophen (the fever and pain reliever). Evodia, mainly through its compound rutaecarpine, may speed up a liver enzyme called CYP1A2, which is one of several pathways that handle acetaminophen. In theory, that could clear it a bit faster and make it slightly less effective for your aches or fever.

Honestly, though, this is based only on test tube and animal research, so the real-world impact in people is uncertain and likely small. The bigger caution with acetaminophen is never doubling up on products that contain it. If you take Evodia regularly, mention it to your pharmacist so they can keep an eye on things. Don't change anything on your own.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

This combo cold product has a few ingredients, but the one that matters most here is dextromethorphan (the cough suppressant). Lab studies hint that evodia may slow down a liver enzyme called CYP3A4, which helps clear dextromethorphan. If that enzyme is blocked, the drug could build up a bit and lead to more side effects like drowsiness, dizziness, or feeling lightheaded.

I want to be honest with you though: this has only been seen in test tubes, not in real people, so the concern is mostly theoretical. Food amounts are unlikely to matter, and concentrated supplements are the bigger question mark. If you're using this cold medicine for a few days, it's worth a quick chat with your pharmacist before adding evodia.

Theoretically, evodia might reduce the levels and clinical effects of CYP2E1 substrates through induction of CYP2E1.
Animal research suggests that rutaecarpine, a constituent of evodia, induces CYP2E1 activity. In rats, rutaecarpine increases markers of CYP2E1 activity, and administration of rutaecarpine with chlorzoxazone, a known CYP2E1 substrate, reduces the area under the curve (AUC) of chlorzoxazone by 84% and increases its clearance by 646%.

In this combo cold product, the piece that matters here is the acetaminophen (the Tylenol component). One of the liver enzymes that processes acetaminophen is called CYP2E1. Lab and animal studies hint that a compound in evodia, rutaecarpine, may speed that enzyme up. The concern isn't that your acetaminophen stops working, it's that a faster-running CYP2E1 can push more acetaminophen down a pathway that creates a liver-stressing byproduct. That theoretically raises the worry about liver strain, especially at higher doses or with alcohol.

Keep in mind this is based only on animal and test-tube data, so it's far from proven in people. The dextromethorphan, guaifenesin, and pseudoephedrine aren't really the issue here. If you take this product regularly, just run it by your pharmacist or doctor first.
