Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the basic idea: your body relies on a liver enzyme called CYP3A4 to help clear Alpelisib (Piqray) out of your system. In lab studies (test tubes, not people), evodia appears to slow that same enzyme down. If that effect carried over into humans, Alpelisib could build up a bit higher than expected, which might mean more of its usual side effects, things like high blood sugar, rash, diarrhea, nausea, and mouth sores.

That said, this is only theoretical right now. It has not been shown to actually happen in people, so I don't want to alarm you. Still, since Alpelisib is a serious cancer medicine, please loop in your oncology team or pharmacist before adding evodia rather than starting or stopping anything on your own.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
