Berotralstat Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the interesting twist with this pairing: the concern isn't really about your Orladeyo (berotralstat) changing. Berotralstat actually acts as a CYP3A4 inhibitor itself, meaning it can slow down a liver enzyme that helps break down evodia's active compounds. So in theory, taking them together could let evodia build up higher than expected.

What this could mean is more of evodia's own effects or side effects, not stronger berotralstat. Keep in mind this is based only on lab and animal studies (using a different drug, ketoconazole), so it's theoretical and not proven in people. Since evodia has limited human safety data anyway, I'd run it by your pharmacist or doctor before combining the two.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the gist with Orladeyo (berotralstat), which you take daily to prevent hereditary angioedema attacks. At higher doses, this medicine can stretch out the heart's electrical reset, what doctors call the QT interval. Evodia has shown in lab dishes and animal studies that it may do the same thing, so stacking the two could, in theory, add up and raise the chance of an irregular heartbeat.

I want to be straight with you: this concern comes only from test tube and animal research, not from people, so it's more of a caution than a proven problem. Still, since both can nudge heart rhythm in the same direction, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you have any heart history.
