Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Rexulti (brexpiprazole): your liver uses an enzyme called CYP3A4 to help clear this medicine out of your body. In lab studies, evodia appears to slow that same enzyme down. If that happened in a person, brexpiprazole could clear more slowly and build up to higher levels than expected. That might mean more of its usual side effects, like drowsiness, dizziness, restlessness, weight gain, or feeling unusually sluggish.

I want to be honest about the strength of this, though. This concern comes only from test-tube research, and it has not actually been seen in people taking these two together. So it's more of a theoretical caution than a proven problem. Still, before adding evodia, please run it by your pharmacist or doctor so they can weigh it for you.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
