Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist with this cough and cold combo. The dextromethorphan piece is partly handled by a liver enzyme called CYP3A4, and lab studies suggest evodia may slow that enzyme down. If that happened in real life, your body would clear the dextromethorphan a bit slower, so it could build up. That might mean more drowsiness, dizziness, or stomach upset, and the chlorpheniramine already adds its own sleepiness and dry mouth.

Now the honest part: this is only test-tube evidence, and it has not been seen in actual people. So I wouldn't lose sleep over it. Still, since both effects can stack, it's worth a quick word with your pharmacist before pairing them, especially if you feel unusually groggy.

Theoretically, evodia might reduce the levels and clinical effects of CYP2E1 substrates through induction of CYP2E1.
Animal research suggests that rutaecarpine, a constituent of evodia, induces CYP2E1 activity. In rats, rutaecarpine increases markers of CYP2E1 activity, and administration of rutaecarpine with chlorzoxazone, a known CYP2E1 substrate, reduces the area under the curve (AUC) of chlorzoxazone by 84% and increases its clearance by 646%.

Here's the honest picture with this combo cough and cold product (the chlorpheniramine plus dextromethorphan). The concern on file is that evodia, through a compound called rutaecarpine, may speed up a liver enzyme called CYP2E1. In theory, revving up that pathway could clear these medicines a little faster and make them work less well, meaning you might get less cough suppression from the dextromethorphan or weaker relief from the antihistamine.

I'll be straight with you though: this is very weak, theoretical evidence based only on animal and lab studies, and CYP2E1 isn't even the main enzyme that handles these two drugs (dextromethorphan leans more on CYP2D6). So I wouldn't lose sleep over it. Still, it's worth a quick word with your pharmacist before pairing them, especially while you're sick.
