Darunavir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the key thing to know: with this combo, it's the Evodia that gets affected, not your HIV medicine. Ritonavir is a strong blocker of a liver enzyme called CYP3A4, and darunavir is boosted by that same effect. When you slow down CYP3A4, the active compounds in Evodia (like evodiamine and rutaecarpine) can build up higher and hang around longer in your body, which may make any side effects or unpredictable effects from the herb more likely.

I want to be honest about the evidence: this is based only on lab and animal studies, so it's more of a theoretical concern than something we've clearly seen in people. Still, since keeping your HIV regimen working matters so much, please check with your pharmacist or doctor before adding Evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for your HIV combo of darunavir with ritonavir. Both of these are handled by a liver enzyme called CYP3A4. In fact, ritonavir is added on purpose as a 'booster' to slow that enzyme down and keep darunavir levels high enough to work. Lab-dish studies suggest evodia may also block CYP3A4 a bit. In theory, that could nudge levels of these drugs upward, which might mean more of side effects like nausea, diarrhea, headache, or changes in blood fats.

The honest truth: this is based only on test-tube research and has not been seen in real people, so it's a soft, theoretical concern. Still, because your HIV therapy needs steady, reliable levels, I'd run any evodia use past your pharmacist or doctor first rather than adding it on your own.
