Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the gist: Evodia may speed up a liver enzyme called CYP1A2, which can be involved in processing hormones in birth control pills like Nextstellis (drospirenone plus estetrol). If Evodia revs that enzyme up, your body could clear the hormones faster, leaving lower levels in your system. For a contraceptive, the worry with weaker hormone levels is reduced reliability (possible breakthrough bleeding, spotting, or even reduced pregnancy protection).

That said, this concern comes only from lab and animal studies, mostly with caffeine and theophylline, not from real-world data with this pill. So it's a theoretical flag, not a proven problem. Since pregnancy protection is the whole point here, I'd still run it by your pharmacist or doctor before combining the two.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the part that surprises people: in this case it's your birth control pill (Nextstellis) that may be acting on the supplement, not the other way around. Drospirenone and estetrol can mildly slow down the same liver enzyme (CYP3A4) that breaks down evodia's active compounds. In theory, that could let evodia build up a bit higher in your body and hang around longer, which might raise the odds of side effects from the herb itself.

I'll be honest though: this is based on lab and animal research only, so it's a soft, theoretical concern rather than something we've clearly seen in people. It doesn't mean your pill stops working. Since evodia hasn't been well studied in humans and you're on contraception, run it by your pharmacist or doctor first before adding it.

Here's the thing to keep in mind with this pairing: the concern runs the opposite direction from what people usually expect. Your birth control pill, Nextstellis (drospirenone and estetrol), isn't the one that gets affected here. Instead, your pill can mildly slow down the liver enzyme (CYP1A2) that breaks down a compound in Evodia called rutaecarpine. In theory, that could let the Evodia compounds build up a bit and have a stronger effect.

I want to be honest about how soft this evidence is. It comes only from lab and animal studies, not real people, so the practical risk is likely small. Still, since Evodia itself is poorly studied, it's worth a quick chat with your pharmacist or doctor before combining them, especially since Evodia isn't recommended in pregnancy.

Your birth control pill Nextstellis contains two hormones, drospirenone and estetrol, and your liver uses an enzyme called CYP3A4 to help break them down. In lab dishes, evodia appears to slow that same enzyme. If it did the same in people, the hormones could linger a bit longer in your body, which in theory might mean stronger effects or more side effects like breast tenderness, nausea, headaches, or mood changes. Drospirenone can also nudge potassium upward, so that is worth a thought too.

Here is the honest part: this has only been seen in test tubes, never in actual humans, so it may amount to nothing. Still, before pairing evodia with Nextstellis, run it by your pharmacist or doctor first.
