Etravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for your HIV medicine etravirine (Intelence). Your liver uses an enzyme called CYP3A4 to help clear etravirine from your body. In test-tube studies, evodia appears to slow that enzyme down. If that held true in people, less etravirine would be broken down, so more could build up in your blood. That could mean a greater chance of side effects like nausea, rash, or headache.

I want to be honest, though: this is theoretical and only seen in lab research, never confirmed in actual patients. Etravirine is not a prodrug, so it doesn't flip the other way. Since your HIV treatment is too important to gamble with, please check with your pharmacist or doctor before adding evodia.

Theoretically, taking CYP3A4 inducers might decrease the levels and clinical effects of evodia.
Animal research shows that concomitant administration of dexamethasone, a known CYP3A4 inducer, with the alkaloid constituents of evodia significantly reduces the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Etravirine can act like a mild booster for the liver enzyme CYP3A4, meaning it can speed up how fast your body clears certain things. The concern on file is that this faster clearance could lower the levels of Evodia's active compounds, making the supplement less effective. Your etravirine itself isn't the one being affected here.

Honestly, this is based only on animal and lab research, so it's quite theoretical. There's no good human evidence, and I wouldn't lose sleep over it. Etravirine is far too important for your HIV treatment to risk anything, so please don't change how you take it. If you're thinking about adding Evodia, just run it by your pharmacist or HIV doctor first.

What Is Etravirine Used For?

Brand name: Intelence

Etravirine (brand name Intelence) is used to treat HIV-1 infection in people who have already received other antiretroviral (HIV) treatments — meaning it is meant for patients whose HIV has had a chance to develop resistance to earlier therapies. It is always used as part of a combination regimen with other HIV medications, never on its own.

Both the brand Intelence and generic etravirine tablets are approved for adults and for children ages 2 to under 18 who weigh at least 10 kg (about 22 pounds) and who have already been treated with other HIV drugs.
